Can Nigeria achieve sustainable improvements in Primary Health Care without transforming the way its health institutions are led?

Despite years of healthcare reforms, substantial investments and renewed commitments to Universal Health Coverage, many Primary Health Care (PHC) facilities in Nigeria continue to struggle with functionality, quality of care, workforce performance, institutional accountability and sustainability. This raises a fundamental question: Are the challenges facing PHC solely about inadequate resources, or do they also reflect gaps in leadership, institutional capacity and accountability?

The reality is that investment alone do not guarantee results. Policies require effective leadership to translate them into action, while institutions need capable leaders to turn resources into functional services and sustainable health outcomes.

Transformational leadership offers an important pathway for addressing these challenges. It goes beyond routine administration and supervision to inspire a shared vision, empower health workers, strengthen institutions, encourage innovation and promote a culture of integrity, accountability and results.

For Nigeria's PHC system, this requires a shift towards seven critical leadership priorities: visionary and strategic leadership; institutional strengthening; workforce empowerment and leadership development; ethical leadership and transparency; innovation and continuous learning; community-centred collaboration; and results-oriented performance management.

An equally important consideration is the need to strengthen existing institutional structures rather than unnecessarily create parallel arrangements that may duplicate responsibilities, fragment coordination and weaken sustainability. New initiatives should complement established institutions, address genuine capacity gaps and reinforce long-term system performance.

Ultimately, transformational PHC leadership must be evaluated by measurable improvements in facility functionality, service availability, quality of care, workforce performance, patient satisfaction, community trust and health outcomes. Leadership success should not be determined merely by programmes introduced, facilities renovated or resources expended, but by the lasting institutional improvements and healthcare services delivered.

Through the Primary Health Care Renaissance agenda, leadership development must become a strategic investment in Nigeria's health system. The proposed PHC Renaissance Thought Leadership Masterclass, through the Health Development Academy, offers an opportunity to translate these principles into practical leadership development, mentorship, institutional problem-solving and performance improvement across national, state, local government and facility levels.

The ultimate challenge is to develop leaders who build institutions rather than personalise programmes, strengthen systems rather than create unnecessary dependencies, and empower people rather than simply supervise them.

Nigeria does not just need more PHC investments and programmes; it needs transformational leaders capable of converting investments into functional institutions, quality healthcare and sustainable results.

As the PHC Renaissance agenda advances, one principle must remain central: The true measure of leadership is not what a leader initiates while in office, but what continues to function, improve and deliver results long after that leader has left.

Strong leadership builds strong institutions. Strong institutions deliver better healthcare. And better healthcare transforms lives.
